Scope of Sessions:
As a supervisor, I will be responsible for overseeing your occupational therapy practice over tele-health, and ensuring that it aligns with the professional standards and regulations established by AHPRA. Our supervision will include regular/one-off meetings, case reviews, and ongoing feedback to support your professional growth and development. As your coach, I am not here to replace any medical intervention. I am here to guide you towards achieving your goals
Schedule
The frequency of our meetings will be determined based on your level of experience and individual needs.
Sessions will be for a duration of 60 minutes but in some circumstances will run over.
Sessions will be held over telehealth or over the phone as required.
Sessions will be scheduled in advance, and may be rescheduled or cancelled as needed by either party with reasonable notice.
Fees
Please see pricing on website for each session. This amount can be paid each time a session is booked.
The supervisor reserves the right to charge an additional fee for any documentation, such as reports, as requested by the Supervisee. The cost will be at the discretion of the Supervisor.
If rates change before this agreement has been signed and dated, the prevailing rates will apply.
Confidentiality
All parties agree to maintain the confidentiality of all information shared during sessions, in accordance with the relevant laws and regulations in Australia.
"Confidential Information" refers to any information shared during the sessions, including but not limited to client information, personal information about the person, and any other sensitive information that is discussed during the sessions.
Obligations: The supervisor/coach agrees to maintain the confidentiality of all information shared during the sessions and not to disclose any confidential information to any third party without the written consent of the person.
Exceptions: The supervisor may disclose confidential information in the following circumstances:
- To comply with legal or ethical obligations, such as mandatory reporting laws or ethical codes.
- To prevent harm to the supervisee or their clients.
- To defend the supervisor against any legal or ethical claims.
